These will help us understand the history and nature of your current condition and speed up the onboarding process. Don’t worry if you do not have a printer. You can fill out the forms when you come in for your appointment. Arrive about 15 minutes early if you have not yet filled out the forms.
Also, make sure to bring in your valid government-issued photo ID as well as your insurance card and any co-payment for your visit. Bring any pertinent medical lab results, X-rays, MRI films, CT scans, EMG, or any referral forms.
