All you will need is a valid government-issued photo ID, your insurance card, and any expected co-payments. We also advise you to bring any pertinent medical history reports and lab results.
If you have a printer, you can also print and fill out the Patient Registration Form and Medical History Form ahead of time. Otherwise, arrive 15 minutes early to fill out the form onsite.
